Medical certificates – France and Italy
New French Alternative – Attestation Santé
As of 2025, some French events have started to accept a self-assessment as an alternative to the medical certificate. Click here to complete the Attestation Santé – you will either need to hold an existing licence with the French Cycling Federation (FFC) or create a new account. Please note, this option is currently only available in French.

Your options to obtain a medical certificate
1. Print out the event’s standard document and take it to your doctor.
2. Use the Destination Sport Experiences online medical certification service.
Option 1: certificate signed by your own doctor
Ahead of making an appointment with your doctor, you should download and print the standard document from the event website. To save you searching we’ve listed a few below.
France
• Etape du Tour
• Marmotte Alps Gran Fondo
• Haute Route Alps
NB: The Paris – Roubaix Challenge DOES NOT require a medical certificate.
Italy
• Gran Fondo Strade Bianche
• Gran Fondo Milan Sanremo
• Gran Fondo Sanremo-Sanremo
• Gran Fondo Lombardia
Option 2:Â use an Online medical certification service.
For some people it may not be that easy to make an appointment with a doctor or your doctor may charge for this. You also may not want to add an extra burden to your local healthcare service as they will prioritize people with a more urgent medical need. That’s why we’re introducing an online medical certification service. Simply fill out the online questionnaire, pay the fee and your certificate will be delivered to you electronically. You will get a generic medical certificate without specific event names on it that can be used as many times as you want within one year of it being issued.
